Target: under 3% of dry weight (about 2-3 kg for most patients). Higher IDWG is linked to hypertension, left ventricular hypertrophy, intradialytic hypotension from aggressive ultrafiltration, and higher mortality. Controlling IDWG is mostly a sodium game - salt drives thirst and fluid retention - so low-sodium eating is the single most effective lever, alongside fluid tracking and the prescribed fluid allowance.
