eGFR is calculated from serum creatinine levels using standardized equations. The two main formulas are: (1) CKD-EPI (Chronic Kidney Epidemiology Collaboration) — the current recommended formula, more accurate at higher GFR levels; (2) MDRD (Modification of Diet in Renal Disease) — older formula, still used in some labs. The calculation accounts for age, sex, and sometimes race. Normal eGFR is ≥90 mL/min/1.73m². A declining eGFR trend over time indicates progressive kidney damage and triggers clinical interventions to slow progression.
