An AV fistula is the gold standard for hemodialysis vascular access. Created by surgically connecting an artery to a vein (usually radiocephal or brachiocephal), the fistula matures over 6-8 weeks as the vein dilates and thickens under arterial pressure. A mature AVF provides high blood flow rates (300-500 mL/min), low complication rates, and can last 10+ years. Monitoring includes physical examination (thrill and bruit), flow measurement, and surveillance for stenosis.
