An arteriovenous graft (AVG) is the second-line vascular access option when native vessels are inadequate for AV fistula creation. Made of polytetrafluoroethylene (PTFE), the graft is tunneled under the skin between an artery and vein. AV grafts have higher complication rates than AV fistulas — including thrombosis (20-60% annual thrombosis rate), infection, and pseudoaneurysm formation — but offer faster maturation (2-4 weeks) and easier cannulation. The Fistula First Breakthrough Initiative (CMS, 2003) targets AVF rates >50% among prevalent hemodialysis patients to reduce AVG and CVC dependence.
