Metformin can be initiated (or continued). Check eGFR at least annually; more often if there is risk of acute decline (dehydration, contrast, new medications).
FDA guidance: do NOT start metformin at eGFR 30-45. If already taking it, continue at reduced dose but reassess kidney function within 3 months. Discuss alternatives (SGLT2i, insulin).
Metformin is contraindicated below eGFR 30 because of lactic acidosis risk. Stop and discuss alternatives with your doctor immediately — do not restart without medical review.
