What is the KDOQI target for Kt/V?

The KDOQI minimum target is spKt/V ≥1.2 per session for thrice-weekly hemodialysis. For twice-weekly schedules, higher targets are needed. URR ≥65% is an alternative measure (Kt/V is preferred).

How is post-dialysis BUN sampled correctly?

Incorrect post-BUN sampling is the most common source of wrong Kt/V results. Use the slow-flow method: stop UF, reduce blood flow to 100 mL/min for 30-60 seconds, then draw. Sampling from the venous line immediately without slow flow overestimates Kt/V.

What if my Kt/V is low?

Common causes: short session time, low blood flow (access problem!), inadequate dialyzer surface area, dialyzer reuse beyond limits, or recirculation. Investigate access function first — a stenosis often underlies sudden Kt/V drops.
