Creatinine 4.0 mg/dL is kidney failure territory (eGFR roughly 12-15) — dialysis or transplant is usually required soon, if not already started.
Creatinine is a waste product produced by muscle breakdown and filtered by the kidneys. It is the most common blood test for kidney function — a rising creatinine usually means falling filtration.
A standard blood test (part of the basic metabolic panel). No fasting required, but heavy exercise, creatine supplements, and a large meat meal the day before can raise it.
