Is dialyzer reuse safe?

When performed correctly per AAMI RD47 standards and manufacturer IFU, dialyzer reuse is safe for most patients. However, reuse is contraindicated in HBsAg-positive patients, patients with active infections, and for high-flux dialyzers used in HDF. Reuse should only be performed by trained technicians with validated equipment and rigorous QC documentation.

What is TCV and why is it important?

Total Cell Volume (TCV) is the volume of fluid that can be held within the hollow fibers of a dialyzer. Loss of TCV indicates fiber bundle clotting. Retiring dialyzers at 80% of original TCV prevents decreased clearance and ensures Kt/V targets remain achievable. A dialyzer losing >20% TCV has significant fiber bundle occlusion.

How should reprocessed dialyzers be stored?

Reprocessed dialyzers should be stored filled with disinfectant at room temperature (15-25°C), away from direct sunlight. The dialyzer should be placed with arterial end upward to maintain disinfectant contact with all fibers. Storage period should not exceed the duration validated for the specific disinfectant (typically 72 hours for Renalin, 7 days for Renew).
